Please use the forms below to submit a Connect to Create request. There are separate forms to request equipment, and to request an arts activity. You may submit one or both.

Once a form is submitted, someone will reach out to you to confirm the availability of your request and to finalize an agreement.

We recommend submitting a request at least a monthy before, if possible.
